Entry header not reprinted on top of a new colunm
In the Index the main entry is reprinted on the next page if the list of sub-entries continues on the next page.
This doesn't happend with colunms, if an the list of sub-entries continues on the next page, the entry isn't put on top of the colunm. Is this a bug or is this done by design?

I did figure out a way to show content from a topic that isn't in the TOC, but I'm not sure that this is also a solution for your problem... I don't think you understand what I mean. I mean for the Title 1, Title 2, Title 3, TOC, Forward, Endnotes, Index and Endnotes 2 sections. And I don't want to include them in or exclude them out of the build just from the TOC. Than you would be able to choose if they are included in the TOC. And wit...
