Lex,

This isn't possible directly at the moment (you might want to post a request in the Wish List section). What you can do is introduce a slight left indent for the Index items -- this looks OK beneath the heading and will also have the effect of separating the two columns.

I would agree that a lot of the dialogs in Help & Manual tend to be very mouse-centric. And I must also admit that I personally didn't even know about the down arrow/up arrow trick. I'm sure the developers will chime in with a comment here... :)
