I often use conditional text tags [If not chm> <End] to put screen shots in the PDF version but omit them from the CHM help. If the tags occur in a numbered list, the number immediately following the <End] tag gets omitted when I generate the CHM. (The PDF seems to work ok.)
The only way I have been able to fix this is to place the cursor immediately after the <End] tag and press Enter. Then press the space bar to add one space character to the paragraph. Then change this paragraph’s formatting to ‘Normal’. Then change its font size to 1 so the extra space is not noticed when you generate the CHM and PDF.

This is a known issue that is difficult to change at the moment. Another way you can fix this is to place a dummy anchor directly after the conditional tag, that will also "protect" the element directly following it.
