Is there a way to add Alt Text to images that will carry over when published to HTML and PDF (for 508 compliance)?

Just enter text in the tooltip field, that will also populate the Alt field (double-click on an image in the editor to edit). You can also automatically populate the Alt field with the image filename if there is no tooltip. You can do this in the Project Explorer on the left, in Configuration > Publishing Options > WebHelp > HTML Export Options.

This worked for generating HTML versions of the file, but the PDF compiler is not picking up the tool tips.

That is normal. PDF only shows tooltips for external links to things like websites. It doesn't show tooltips for internal links. They are also automatic, generated by Adobe Reader.
