I see the index in the .chm, but it doesn't appear in the pdf. what am I doing wrong?
I am not modifying any of the standard print manual templates.

Hi Pat,
When you say "index" do you mean the keyword index or something else? Whether or not the keyword index is generated in PDF depends on whether its page is activated in the PDF template. Open the template in Manual Designer and check the Keyword Index tab. If it is grayed out, activate it by checking the "Print this section" option in Pages > Page Options. You may also need to activate or add a reference to it in the Table of Contents section.

The Table of Contents does list the Index, with a page number of zero.
Interesting - when I drill down : Configuration, Publishing Options, Adobe PDF, then
Click the Print Manual icon at the top, I get choices to Print or Preview,
If I choose Preview, it appears.
Here is the Publish Format screen:
Any suggestions?

When you say "print" I assume that you really mean "publish", and not print to paper. If you're getting a page number of zero for your keyword index in your PDF output then the keyword index isn't activated. One possibility is that you are editing one copy of the template and actually using another one. Your screenshot shows that you are using a template in the Templates folder in the Help+Manual program folder, and it's not possible to edit templates there -- that's why they are stored there. So if you have created an edited version somewhere else you need to click on the Select Print Manual template bar, then select the Open Skin... option right at the top of the large menu displayed and navigate to the edited copy and select that.
