Hi Mark,
If you don't want those things then don't use a Premium Pack skin. Use one of the basic standard skins. However, there too you will also have basic topic navigation in the topic header and you really shouldn't try to remove it because it will make accessing your documentation a lot easier...
If you don't want any navigation at all for some arcane reasons you can achieve that best by not using a skin at all. Then you will be using the standard topic page template in your project instead. Then go down to Configuration > HTML Page Templates > Default in your project. If you see the source code tab active on the right click on the Reset Template button to delete the source code modifications and activate the simple mode tab. Then you will be able to remove the navigation options and you'll have basic CHM pages with just a plain header and no navigation at all.
