Keyword indexing for TOC links

At present it is possible to create a TOC item which links directly to an external web page, or opens a file via the exec: function.

But unless I'm missing something, there is no way to add keywords for this TOC item because there is no corresponding topic file.

It would be very useful to be able to reference such TOC items in the keyword index, so that clicking the entries in the index is equivalent to clicking the TOC item.

Or alternatively to create such index entries via the Index Tool, with the same link functionality (including the exec: option).
