I set all my Webhelp output to use the .php extension in HTML Export Options.
(This is the only way to be absolutely sure content won't get cached by any known browser.)
However, this option is not honoured by embedded HelpXplains, which are created as .html files.

Xplains are separate entities, created by a separate program. They have nothing to do with Help+Manual itself directly. Similarly, Help+Manual wouldn't change any file extensions etc. in embedded YouTube videos.

Well I agree, up to a point. That's what you would expect if creating Xplains directly and uploading them.
But when they are inserted in a topic directly in HM8, compiled there, and included in the project files on upload, it seems to me that the HM8 export settings ought to be honoured.
Alternatively, is there a setting for the output extension in an Xplain itself?

That's really what you are doing. When you embed an Xplain it is compiled by HelpXplain, not by Help+Manual, when you publish your project. The only two things Help+Manual does is send the command line to HelpXplain to perform the compile, and insert the embed code in the page.That's what you would expect if creating Xplains directly and uploading them.
HelpXplain doesn't currently support alternative file extensions though. I suppose that might be added in the future, but it's not currently available.
