Found this error.
There is project 1, it has a picture named trmd6img1.png
There is project 2, which has a different picture, but also named trmd6img1.png
There is a project in which project 1 and 2 are combined. When you export a project to Word or PDF, only the second picture remains.
An example of an error follow the ling (with a word file). It appears on the latest version 8.3.1

The error is that the two pictures have the same name. When you combine the two projects as modules then it is the same as a single project. Help+Manual finds image files by looking through all the folders in the project search path from top to bottom and then inserting the FIRST file that it finds with a matching name. This makes image file management very flexible, but it also means that you must avoid duplicate image file names.
If you have two images in different folders with the same name then only the first one will be used. And even if the two projects do insert different images when they are combined, they both have the same name, so the second one will overwrite the first one in the combined output. This also applies to images with the same name but different formats, for example introduction.jpg and introduction.bmp, because images often need to be converted to different formats for different output types. Then they still end up with the same name and overwrite each other.
What this means is that you need to change the name of one of the images and re-insert it with the new name. That will solve the problem.

Could we not have an override option in future for the image to be accessed from the specified folder?
We have multiple screens in our manuals from multiple sources and I constantly have to change filenames. This really complicates things when screens are updated. One of my main H+M sells was that engineers could just update screens and they would automatically update in the manual.

No, because then you would have to store the location of the file in the topic. That is strictly forbidden as it would undermine the entire concept of how external assets are managed in Help+Manual. There is literally no chance of that changing. Zero.
