These are not really questions about Help+Manual but about structuring your help itself, and those are not questions that can even be answered in a general sense. In all cases the answers will depend to a great extent on your application and your specific needs. Questions like "how to write user-friendly instructions" cannot really be answered at all in general terms. You would have to provide a specific example, and generally this is something you have to learn by experience.
