I erased a bookmark in a topic but the thumbtack remained in the TOC. I have attached the .HMX file with the single topic.
Note the [Command] picture in the topic header. When preparing the file to send to you I erased the unlinked hotspot from this graphic. This solved the problem !!! (The attached file shows the thumbtack).

"Repair and Recover" (which, among other things, includes a re-build of the bookmarks list) was the only thing that worked for me.
